Residential Habilitation and Support Services
Residential Habilitation and Support Services are a goal-based service available through the Community Integration and Habilitation (C.I.H./ Waiver), which provides up to 24-hour daily support to help individuals achieve their maximum potential for independence and acquire skills necessary to successfully live in their own homes. These services include personal care, medical appointments, meal preparation, shopping, and transportation, and they can be provided in the individual or family home or in the community, depending on the level of service needed. The service is available in two levels based on the approved budget and can be provided by a primary caregiver or guardian for more or less than 35 hours per week.
Day Habilitation
Community-Based Habilitation-Individual is a goal-oriented service provided outside of the individual’s home, aiming to improve daily living skills, socialization, communication, and community integration. This service is available through the Community Integration and Habilitation and Family Support Waiver and can be provided in the individual’s home, family home, or community. The service has two levels based on budget and can be provided by a primary caregiver or guardian as long as the waiver individual is 18 years of age and services does not exceed more or less 35 hours per week.
Participant Assistance and Care Services
The Participant Assistance and Care Services aim to assist participants in their daily living activities and enable them to live successfully in their own homes and communities. These services, available through the Family Support (F.S.) Waiver includes hands-on assistance, reminders, supervision, and monitoring to ensure the participant’s health, safety, and welfare. They cover personal care, mobility assistance, scheduling appointments, shopping, leisure activities, correspondence, and bill paying. The primary caregiver or guardian can provide the service if the participant is over 18 years old.
